When booking your appointment, you will need provide a credit card for payment and you will be sent a medical history intake form that must be filled out prior to the appointment. A bloodwork requisition will also be sent to you depending on the type of service you book. This bloodwork must be completed at least one week before your appointment.

If an appointment is missed or cancelled within 72 hours, there will be a charge of $100 amount on the credit card on file. This is to ensure that the clinic is running smoothly and other patients have access to care.
